A small metal and polyethylene implant resurfaces only the medial compartment. 
  • Patellofemoral replacement: For isolated patellofemoral arthritis – pain specifically behind the kneecap during stair climbing, squatting, and prolonged sitting – a patellofemoral arthroplasty resurfaces the undersurface of the patella and the trochlear groove, leaving the tibio-femoral joint untouched. 
  • The advantage of preserving bone: A partial knee replacement removes substantially less bone than a total replacement. This matters for future revision surgery – if a partial replacement eventually needs to be converted to a total knee replacement years later, there is more bone stock to work with. 
  • Patient selection is critical: Partial knee replacement produces excellent results in the right patient and poor results in the wrong one. Intact cruciate ligaments, a correctable deformity (not a fixed varus), a BMI that isn’t severely elevated, and genuinely isolated compartmental disease are the requirements.
