Are antibiotics needed for a cough and cold?
In the vast majority of cases – no. Acute respiratory tract infections are predominantly viral. Antibiotics do not treat viral infections. What causes chronic cough after a cold?
Post-viral cough – also called post-infectious cough syndrome – is caused by airway inflammation and temporary cough receptor sensitisation after a respiratory infection. It resolves spontaneously within 6 to 8 weeks in most cases. If it does not, investigation for post-nasal drip, cough-variant asthma, or GORD is warranted.
Can GORD (acid reflux) cause a chronic cough?
Yes – and it is one of the most commonly missed causes of chronic cough. Gastro-oesophageal reflux can trigger cough by direct acid contact with the upper airway, or by a reflex mechanism even without oesophageal symptoms. Many patients with GORD-associated cough do not experience heartburn – the cough is their only symptom.
What is ACE inhibitor cough and how is it managed?
ACE inhibitors – lisinopril, enalapril, ramipril – are widely used for hypertension and heart failure. They cause a dry, persistent cough in 10 to 15% of patients – more commonly in people of South Asian ethnicity, including Gujarati patients. The only treatment is switching to an ARB (angiotensin receptor blocker). If you develop a persistent cough after starting one of these medications, mention it to your prescribing doctor.
My child has had a cold and cough for 3 weeks - should I be worried?
Children are more susceptible to respiratory infections and can take longer to recover – particularly in high-pollution urban environments like Ahmedabad.
