The Injury That People Take Less Seriously Than They Should

In my clinic at EPIC Multispecialty Hospital, Ahmedabad, one of the most common histories I hear is a patient who twisted their ankle — during cricket, badminton, a morning run, or simply stepping off a kerb — and was told to rest, apply ice, and wait. Weeks later, the ankle is still sore, still slightly swollen, and giving way intermittently. They have come to see me because it has not got better the way it was supposed to.

Ankle sprains are the most common sports injury in the world. They are also one of the most undertreated. The “rice and rest” approach — rest, ice, compression, elevation — is appropriate for mild sprains. But a significant proportion of ankle injuries involve partial or complete ligament tears, and these need a different approach from the outset if they are to heal properly and not become a source of chronic instability.

This article is my attempt to help you understand what your ankle injury is — and what it needs.

The Anatomy — What Sprains and Tears Actually Involve

The ankle joint is stabilised by a complex of ligaments on both sides. The most frequently injured are on the outside (lateral side) — the ATFL (anterior talofibular ligament) and CFL (calcaneofibular ligament). These ligaments are stressed when the foot rolls inward — the classic ankle sprain mechanism.

A mild sprain stretches the ligament fibres without tearing them. Moderate sprains involve partial tearing. Severe sprains involve complete rupture of one or both lateral ligaments.

The problem is that all of these injuries look similar from the outside at first — swelling, bruising, and pain on the outer ankle. A clinical examination by a specialist and, when needed, an MRI scan is what accurately characterises the injury. Treating a complete ligament rupture the same way as a mild stretch leads to inadequate healing and chronic instability — where the ankle continues to give way months or years after the original injury.

How I Assess an Ankle Injury

When a patient comes to me at EPIC Multispecialty Hospital, Ahmedabad, with an ankle injury, my assessment follows a specific sequence.

First, a clinical examination — feeling for point tenderness over the ligaments and bones, testing ligament integrity with specific stress tests (anterior drawer test and talar tilt test for the lateral ligaments), assessing range of motion and any signs of joint effusion.

X-rays are taken to exclude fractures — particularly the fibula and the base of the fifth metatarsal, both of which can be fractured in ankle inversion injuries and are easily missed if the ankle is simply treated as a sprain without imaging.

If clinical findings suggest significant ligament damage — severe pain, instability on stress testing, significant swelling — I arrange an MRI scan. This shows precisely which ligaments are involved, the degree of tearing, and any associated injuries to the cartilage inside the joint.

The Ottawa Ankle Rules — a validated clinical decision tool — help me determine when X-rays are necessary and when the risk of fracture is low enough to proceed without them. I apply these consistently in my assessment.

Treatment — From Mild Sprain to Ligament Reconstruction

For mild sprains — stretched ligaments with intact fibres — RICE in the first forty-eight hours followed by early guided movement and physiotherapy produces excellent outcomes. The emphasis is on proprioceptive rehabilitation — retraining the ankle’s ability to sense its position in space, which is often disrupted even by mild sprains and contributes to re-injury risk if not addressed.

For moderate sprains with partial tearing, a period of functional bracing — a semi-rigid ankle brace that allows movement while protecting the ligament — combined with physiotherapy produces good healing and return to function in four to six weeks.

For complete ligament ruptures, non-surgical management can still produce adequate stability in many patients — particularly those who are not involved in high-demand sports. The key is an extended physiotherapy programme and appropriate bracing through the healing period.

For athletes in Ahmedabad who have complete ligament ruptures and significant functional instability despite adequate rehabilitation — or for patients who have developed chronic ankle instability from multiple sprains — surgical ligament reconstruction (Brostrom repair) restores anatomical ligament integrity and eliminates the instability. This is performed arthroscopically at EPIC Multispecialty Hospital, Ahmedabad, through small incisions, with a return to sports typically at four to six months.

Chronic Ankle Instability — When the Sprain Becomes a Chronic Problem

Chronic ankle instability is an underrecognised condition in Ahmedabad. It develops in patients who had a significant ankle sprain that was not adequately rehabilitated — leaving the stretched or torn ligaments to heal in a lengthened position, without the tensile strength to reliably stabilise the joint. The result is an ankle that gives way repeatedly — with subsequent sprains causing cumulative damage to the cartilage and increasing the long-term risk of ankle arthritis.

If your ankle has given way more than three times, if you feel it is unreliable on uneven ground or during sport, or if a sprain from more than three months ago still gives you intermittent symptoms — please come for a proper assessment. Chronic instability is very effectively treated, either with a structured physiotherapy programme for mild cases or with surgical Brostrom repair for significant ligament insufficiency.

The ankle does not have to be something you manage around indefinitely. It can be fixed.

Common Questions Patients Ask

How do I know if I have fractured my ankle or just sprained it?

Fractures and sprains can feel very similar initially. The Ottawa Ankle Rules help clinically — tenderness over specific bony areas (the tip or back of the fibula, the navicular, or the base of the fifth metatarsal) indicates X-ray is needed. If in doubt, please come for an assessment. Missing a fracture and treating it as a sprain leads to malunion and long-term problems.

Should I walk on an ankle sprain?

For mild sprains, protected weight-bearing with a brace from early on is generally better than complete immobilisation — it promotes appropriate ligament healing and faster recovery. For severe sprains or suspected fractures, rest and medical assessment are the priority. Do not push through significant pain.

How long before I can play sport after an ankle ligament tear?

Return to sport depends on the severity of the injury and the demands of the sport. Mild sprains may allow return in two to three weeks. Complete ligament ruptures treated non-surgically require six to eight weeks before return to sport. After surgical Brostrom repair, return to competitive sport is typically four to six months.

Can old ankle sprains be treated years later?

Yes. Chronic ankle instability from old poorly-healed ligament injuries can be treated with Brostrom ligament repair even years after the original injury. The condition may have allowed cartilage damage to accumulate in the meantime, which is assessed at the time of surgery and treated simultaneously. Earlier treatment produces better results, but it is never too late to address significant chronic instability.

Is ankle surgery available at EPIC Hospital Ahmedabad?

Yes. EPIC Multispecialty Hospital, Ahmedabad, performs arthroscopic and open ankle surgery — Brostrom ligament repair for chronic instability, ankle arthroscopy for impingement and cartilage lesions, and reconstructive procedures for complex ankle conditions.
